Specialized Alternatives for Families and Youth (SAFY) of America is a leading national nonprofit organization providing a full continuum of services for children and youth in the child welfare and juvenile justice systems. Specialized Alternatives for Families and Youth (SAFY) is a not-for-profit (501)(c)(3) agency that provides comprehensive treatment services in Alabama, Indiana, Kentucky, Nevada, Ohio, Oklahoma, South Carolina and Texas.
We currently have four Adoption Service Coordinator positions available in Indiana: Lafayette, Merrillville, Muncie, and South Bend.
Our Adoption Services Coordinator is responsible for executing our mission while locating and/or delivering strength based, family-centered adoption and culturally competent services. Facilitate the opening and closing of the case and oversee transition to an on-going care plan, using resources and available flex funding to assure that services are based specifically on the needs of the child and family. Each referred family receives access to services through an Adoption Services Coordinator acting within a team, with availability 24 hours a day 7 days a week.
This will be accomplished through the following:
•Evaluate and interpret all strength based assessments administered through the Child and Adolescent Needs and Strengths (CANS) assessment tool and assist the family in all aspects of family development and adoption preservation, including the ability to interpret psychiatric, psychological and other evaluation data and use information in the formation of a collaborative plan of care. The plan of care will address the need for revisions, adjustments and monitoring of crises so that progress can be evaluated, ensuring that the philosophy of care is consistent and that there is progress toward service goals.
•Ensure that an on-going dialogue is established with all referral families and that parent and family involvement is maintained throughout the service period. Assist the family in convening a team, following the DCS practice model, to include other family members, service providers and informal supports to for a plan of care with clearly defined goals. Maintain creativity, flexibility and optimism about the strengths of child and their families and ensure that all goal setting and service plans are mutually established between the client, Adoption Services Coordinator, and family with a written report submitted to the DCS referring worker within seven (7) days of the initial face-to-face intake. Goals are family driven, behaviorally specific, team developed that are measurable and attainable.
•Complete home studies for private adoption families after three home visits/family interviews. Assist as needed in completing home studies for potential SAFY foster families.
• Complete child summaries as requested.
•Complete all documentation using a computerized clinical record and maintain central file which will consist required paperwork such as treatment summaries, payment and resource utilization records, case notes, legal documents and releases of information. Maintain communication with DCS, including timely monthly progress reports and any other documentation arranged by DCS.
Educational Requirements:
•Bachelor's degree in social work, counseling, psychology, sociology, child & family studies or other related human service degree is required.
Experience Requirements:
•Three years of experience in working with domestic and/or international adoptions is required.
